Virtual Reality in Eating Disorders and Obesity: Different new technologies have been introduced over the last few years that are increasingly finding application in health care delivery for patients with eating disorders and obesity. These include self-help (supervised and unsupervised), telemedicine, telephone therapy, e-mail, Internet, computer software, CD-ROMs, portable computers, and virtual reality techniques. One of the most promising is virtual reality (VR), an advanced form of human-computer interface that allows the user to interact with and become immersed in a computer-generated environment in a naturalistic fashion. Distorted body image, negative emotions and lack of faith in the therapy are typical features of these disturbances and are the most difficult characteristics to change. One innovative approach to their treatment is to enhance traditional cognitive behavioral therapy (CBT) with the use of virtual reality. During the VR sessions they use the “20/20/20 rule.” During the first 20 minutes, the therapist focuses on getting a clear understanding of the patient’s current concerns, level of general functioning, and their experiences related to food and to the body. This part of the session tends to be characterized by patients doing most of the talking, although the therapist can guide with questions and reflection to get a sense of the patient’s current status. The second 20 minutes is devoted to the VR experience. During this part of the session the patient enters the virtual environment and faces a specific critical situation such as a kitchen, supermarket, pub, restaurant or gymnasium. Here, the patient is helped in developing specific strategies for avoiding or coping with the negative emotions induced by the situation. In the final 20 minutes the therapist explores the patient’s understanding of what happened in VR and the specific reactions–emotional and behavioral–to the different situations experienced. If needed, some new strategies for coping with the VR situations are presented and discussed. To support the empowerment process, the therapists follow the Socratic style–they use a series of questions related to the contents of the virtual environment to help clients synthesize information and reach conclusions on their own.

 

 

Paths to first treatment and duration of untreated illness in anorexia nervosa: are there differences according to age of onset?  This study examined paths to first treatment and the duration of untreated illness in 140 anorexia nervosa patients using validated questionnaires and a clinical interview. The differences between individuals with an early (≤14 years, n = 40), intermediate (15-18 years, n = 53) and late onset (≥19 years, n = 47) were investigated. Participants were most commonly informed about their diagnosis and first treatment facility through general practitioners and pediatricians. The duration of untreated illness exceeded 2 years in the complete sample (25.14 months) and was longest for individuals with an early onset. The early onset group was more often externally vs. internally motivated and more frequently informed about treatment options by their social network, e.g. parents, than patients with a late onset. The results emphasize the relevance of training general practitioners and pediatricians about anorexia, the need to include parents and teachers in eating disorder prevention and to improve targeting young individuals in early interventions. Contemporary criteria of the diagnosis and current recommendations for nutritional therapy in anorexia nervosa. The basic criterion for the diagnosis of anorexia (AN - anorexia nervosa) by ICD-10 (International Classification of Diseases, version 10) is the body weight less than 15% of the expected normal body weight. According to DSM-IV (Diagnostic and Statistical Manual for Mental Disorders, version IV) the basic feature of AN is a refusal to maintain body weight equal or greater than the minimal normal weight. The prevalence of anorexia nervosa is 0.3-0.5% or even 1.3-3.7% if include pre-anorexic states (eg. the phenomenon of pro-ana). The main feature of anorexia is a reduction of caloric intake. According to the recommendations of the American Psychiatric Association (APA) for nutritional treatment of patients with AN the main goals in therapy of AN are: restoration of body weight, normalization of eating patterns, achievement a normal feeling of hunger and satiety and correction of the consequences of improper nutrition. APA suggests that achievable weight gain is about 0.9-1.4 kg per week in the case of hospitalized patients and approximately 0.23-0.45 kg per week in the case of outpatients. During the nutritional treatment of AN numerous side effects including anxiety, phobia, occurrence of obsessive thoughts and compulsive behavior, suicidal thoughts and intentions may occur. According to National Institute for Clinical Excellence (NICE) the most important goal of AN therapy is weight gain in the range of 0.5-1 kg per week in hospitalized patients and 0.5 kg per week for outpatients. A person suffering from anorexia in the initial period of nutritional treatment spends twice more energy to maintain elevated body temperature, which significantly increases during the night rest. This phenomenon is called nocturnal hyperthermia and has a negative effect on the healing process. "Refeeding syndrome" is an adverse effect of nutritional treatment in anorexia. It is caused by too rapid nutrition in a patient suffering from chronic starvation. It can endanger the patient's life. Singing Sensation Kesha opened up about seeking professional help for an eating disorder in the latest issue of Teen Vogue. The 27-year-old star said, "My whole message is to love who you are and accept all your beautiful imperfections. When I felt I was slipping into unloving territory with myself, I knew I had to listen to my own advice and correct it…I felt I needed to get help, not only for myself but also for my fans. My worst fear in life is to be fake." Kesha also revealed that being in the entertainment industry, where she's constantly photographed and scrutinized, had only intensified her insecurities and body image issues. "It really messed with my head, and I realized I couldn't do it by myself. The decision to take control of it is the scariest thing I've ever done, and this is coming from someone who dives with sharks and jumps out of airplanes for fun." But during her two-month stay in rehab, Kesha says she wrote 14 new songs. EATING DISORDER BUSINESS REPORT: Private Equity Investor Interest in Eating Disorder Treatment. The lack of dominant businesses in eating disorder treatment has attracted private equity firms interested in acquiring small to mid-sized companies and building them up through acquisitions. McCallum Group Inc., a provider of in-patient and out-patient treatment in St. Louis and Austin, Texas is seeking to capitalize on that trend by putting itself up for sale. Described by market research firm IBIS World as “fragmented,” with a large number of small-scale establishments, the eating disorder treatment industry is estimated to have grown to $2.7 billion in 2013. The National Eating Disorder Association cites research estimating that nearly 30 million American men and women suffer from eating disorders such as anorexia nervosa, bulimia nervosa and binge-eating. Treatment centers such as McCallum, categorized as behavioral health service providers, have benefited from the tailwinds of the Affordable Care Act and the updated Mental Health Parity and Addiction Equity Act of 2008, which puts mental health coverage on par with that of physical maladies. Such businesses may also benefit from the Affordable Care Act’s extension of the amount of time children can stay on their parents’ insurance, given that anorexia typically appears in early to mid-adolescence. Previously children were only eligible to stay on their parents’ coverage until age 18, now the limit is 26. But some PE firms ventured into the space long before those developments. Trinity Hunt Partners bought St. Louis-based Castlewood Treatment Center, which focuses on psychologically acute patients in eating disorder, in 2008 and was also an owner of Denver’s Eating Recovery Center between 2010 and 2013 before selling it to Lee Equity Partners. Eating Recovery Center treats patients who are suffering physically from physical disorders like anorexia and bulimia. Webster Capital, meanwhile, in 2011 invested in Discovery Practice Management, a Los Alamitos, Cali.-based operator of residential treatment programs for adolescents and women suffering from eating disorders and substance abuse or emotional disorders.

Training In Transdiagnostic CBT (CBT-E) For Eating Disorders. The Centre for Research on Eating Disorders at Oxford (www.credo-oxford.com) is able to offer free online training in CBT-E to eligible therapists who are willing to take part in a study evaluating a new form of training. A detailed and clinically-rich training website has been developed that provides a very detailed practical description of how to implement CBT-E (delivered by Prof Chris Fairburn) together with a large number of clinical demonstrations and learning exercises. The website has been designed to be used with or without accompanying guidance (provided over the phone or via Skype). San Diego, CA: Treatment Center News - UCSD Eating Disorder Center. The Center focuses on developing treatment based on empirically supported modalities. UCSD offers Partial Hospitalization and Intensive Outpatient programs for adolescents and adults, both males and females. The adult programs place a heavy emphasis on Dialectical Behavioral Therapy with support from Cognitive Behavioral Therapy. The adolescent programs are based in Family Based Therapy, as well as Cognitive Behavioral Therapy. Each of the outpatient programs provides group and individual therapy, dietary support, medical management, psychiatric assessment and treatment of co-occurring disorders. Eating Recovery Center Set to Open New Residential/Partial Hospitalization Treatment Facility in Denver. Eating Recovery Center, an international center of excellence in the treatment of eating disorders, announced the opening of its sixth Denver facility. Denver, CO: Treatment Center News - The Eating Disorder Center of Denver (EDCD). Established in 2001, the Eating Disorder Center of Denver is one of the nation's foremost centers for the diagnosis and treatment of the full range of eating disorders. EDCD is committed to empowering males and females 18 years of age and older suffering with anorexia, bulimia, binge eating and related disorders to help them achieve and sustain recovery. A multidisciplinary team of psychiatrists, physicians, clinicians, family therapists and registered dietitians work together to create a specialized, evidence-based treatment plan for each patient. The Partial Hospitalization Program at EDCD operates 10 hours per day, 7 days per week and includes supervised meals and snacks. Insight Behavioral Health Centers Adds Physicians to Support Expanded Medical and Behavioral Non-Diet Weight Management. In order to better serve existing clients and additional patients, Insight Behavioral Health Centers announces the expansion of its Adult CORE program and the launch of a new, highly specialized adolescent intensive weight management treatment and prevention program (AP CORE). In addition, two of Chicago’s leading obesity medicine physicians have joined Insight’s medical leadership team to oversee this expanded, medically supported programming for both adults and adolescents.
